Through our partnerships with skilled nursing facilities, we are able to provide acute care as well as a consistent physician presence to our valued patients and their families. Brian Chase, M.D.

Brian Chase, MD attended Binghamton University, graduating with a degree in Biology. From there, he earned his medical degree from SUNY Upstate. He completed a residency in Family Medicine from St. Joseph Hospital Syracuse, NY.

Dr. Chase worked as Hospitalist for 8 years prior to his move in to Transitional Medicine. He is board certified in Family Medicine.

“We have come to a crossroads in medicine.  We lie at the intersection of a more progressive and technical system of health care delivery.  All this while still trying to maintain a relationship with patients.  Transitional medicine, as it has come to be referred, is an attempt to transition patients and families from an often overwhelming acute care experience, to home or a higher level of care.  That often means taking a more existential view of ones medical care and life goals.  This time in ones life has often been overlooked in the medical community.  I believe we have entered an era where transitional medicine will take center stage in the care of our aging population.”

John Pylman, M.D.

John Pylman, M.D. completed his undergraduate coursework at Duke University and later received an M.S. in Physiology and Biophysics from Georgetown University. He attained his medical degree from George Washington University and attended the Family Medicine Residency Program at St. Joseph’s Hospital Health Center in Syracuse, NY. Regarding his decision to enter primary care, Dr. Pylman recalls “Family Medicine has allowed me to enter health care at a pivotal moment in the delivery of medicine in this country. More than ever, patients and families are expecting prompt delivery of appropriate treatment from providers they know and can trust. Meanwhile, the complexity of our medical system is at an all time high, which often serves as a barrier to care.” This lead him to adapt his practice to address patients who were chronically ill and most vulnerable, he says. “In the nursing home environment, we see patients for whom acute treatment and early intervention is an absolute necessity. By delivering an inpatient level of acuity to the long term and rehabilitation settings, we’re often able to address chronic medical comorbidities as well as minimize complications and unnecessary hospitalizations.” Dr. Pylman is a Diplomate of the American Board of Family Medicine.
